Coastal Surgical Center, LLC must inform you that it DOES NOT honor the terms of the Advance Directive during your stay at our facility.

In accordance with federal and state laws, a patient or, as appropriate, the patient’s representative has the right to make informed decisions regarding the medical care the patient receives.  Furthermore, all patients are informed of their right to change their provider if other qualified providers are available.

If a patient is adjudged incompetent under applicable State laws by a court of proper jurisdiction, the person appointed under state law to act on the patient’s behalf exercises the rights of the patient.  If a state court has not adjudged a patient incompetent, any legal representative designated by the patient in accordance with State law may exercise the patient’s right to the extent allowed by State law.

An advance directive is a legal document that gives you direction or wishes as to your medical care, should you become unable to make or communicate your decisions concerning your medical care. 

A copy of the “Caring Connections Program”, the New Hampshire Advance Directive, is available at our facility for your review.  You may also preview and/or print a copy of this document prior to your arrival at the facility by visiting the following link on the web: https://www.healthynh.org/images/PDFfiles/advance-directives/2017_ACPG_Final.pdf

If a patient should provide his or her advance directive, a copy will be placed in the patient’s medical record.  In case of emergency, you will be transferred to the hospital.
